GENERAL INFORMATION

APPLICANTS FOR GRADE VII QUALIFYING TEST

DAY & DATE OF EXAMINATION:March 23 – 24, 2019

TIME:6:30 – 12:00 A.M. (Morning Batch)

12:30 – 5:00 P.M. (Afternoon Batch)

PLACE OF EXAMINATION:  CCIT Function Hall

DEADLINE FOR FILING OF APPLICATION FORM:      March 20, 2019

QUALIFICATIONS OF AN APPLICANT:

To qualify for the examination, the applicant must:

  1. be a Grade Six (6) pupil expecting to graduate at the end of the School Year with a general average of  80% or higher based from the last grading period preceding the filing of the Application Form
  2. be an elementary graduate who has not yet entered high school and with a general average of 80% or higher

DOCUMENTS TO BE SUBMITTED:

  1. Correctly and completely accomplished Application Form
  2. Photocopy of Birth Certificate issued by the PSA (bring original copy)
  3. Two identical 2”x2” recent photographs (colored in red background with name tag), one to be attached to the Application Form and the other to the Test Permit
  4. Photocopy of Form 138 duly authenticated by the School Principal (bring original copy)
  5. A self-addressed and stamped envelope for the mailing of the examination result
  6. A non-refundable testing fee of PhP180.00

PROCEDURE IN PROCESSING APPLICATION FORMS:

The applicant will:

  1. Secure Application Form from the UNP Testing Services (UTS) Office or download it at UNP Testing

      Services (Facebook page)

  • Fill out the Application Form and complete the requirements
  • Proceed to UTS for final processing of documents
  • Pay the prescribed testing fee at the Cashier’s Office
  • Submit Official Receipt at the UTS for the preparation of test permit
  • Get test permit
